To qualify for the draw, fill out this form providing us with names and contact numbers of 5 family members, friends, or colleagues.

Competition rules

1. The people you refer to us must be contactable on the number supplied and must verify that they know you when contacted.
2. You must have an active policy at the time of the draw.
3. You may submit additional entry forms provided each additional entry does not contain names already submitted.
4. The decision of Legal&Tax will be final and no correspondence will be entered into.
5. Lucky draws will take place on the last Monday of February, April, June, August and October.
6. Winners will be contacted within 10 working days after each lucky draw.
7. If we are unable to contact you within 10 days from winner selection on your primary cell number, which has been provided by you to us, we will proceed to select a new winner.

8. By entering Fortune 5 competition, you agree to Legal&Tax disclosing your name and surname during the announcement of the competition winners, if and where applicable.
